Blood Drive Donation Info/Sign Up – October 20th

Attached is the Blood Drive Sign-Up form, for our upcoming Blood Drive, next Friday. the 20th, in the Center Street Gym from 10am-4pm.

Our goal is 70 donations, which is 20 more than last year, so WE NEED YOU to help save lives across Wisconsin by donating blood. You must be at least 16 years old to donate, and if you are 16, you will need a signed Parent consent form to donate; it is attached below. You can also stop into room B127, Mr. Omo’s room, for a paper copy to take home.  If you’re 17 or older, no form is needed.

Complete the sign-up form here so we can sign you up for a time slot. Donations will take less than an hour and should be done over your Lunch and Learn time and/or study hall, but we can accommodate a donation during part of class time so long as your teacher signs off on it.
